Death Certificate in Punjab Pakistan

Local Government and Community Development (LG & CD) is the relevant department for Death Certificate. Relevant Union Council issues Death Certificate. A person can go directly to the concerned Union Council with required documents to get Death Certificate. Normally a Union Council issues death certificate in three days.

If you go to the e-Khidmat Markaz of Punjab Government you can obtain the certificate in three days after payment of 100 rupees.

Documents required getting Death Certificate

  • You need an application Form which are available in Union Council and also available in the e-Khidmat Markaz (Attested)
  • Death Certificate of Hospital if death in Hospital
  • Affidavit if Death is in House
  • Bank Challan Form

If you go to e-Khidmat Markaz of Punjab Government you only need to fill the form and pay the 100 rupee fee on bank challan form along with Hospital Death Certificate (if death in hospital) to get the Death Certificate. If you go to concerned Union Council you may need all above mentioned documents.
