History of hair transplant | The first sparks …

In 1822, in Würzburg, Germany a doctor named Difenbach, took the first and most successful steps in the development of hair transplantation. However, the results of studies in those years were very different from today’s methods. He began his research by experimenting with animal skins and feathers. Then, in the first step, he grafted a part of his hair to his arm.

But someone took the second step about a century later in a far far country! Japan. In the late 30s a Japanese surgeon invented a method that nowadays we know it as the first method in the history of hair transplant. The start of Follicular unit extraction or FUE method, goes back to Japanese articles.
Shuji Okuda reported that you can nucleate a follicle inside the donor site; and also detach it from it’s upper appendage. According to the release date (1939), the war years limited the spread of technology. It was clever that Okuda determined that they only needed the upper third of the hair follicle for nucleation; but he failed to pull some hair transplants from their original location.

Therefore, he felt that it was important to “grab” the deep sections of the hair follicle, indicating that he did not realize that he could completely remove the hair from the donor area in patients. In the late 1980s, Pascal Bojma pioneered mechanical punching, eventually introducing his own trading system called Calvitron®, which received patents for his device from the United States and around the world.

Development until inventing new techniques

In 1996, Bojma began extracting follicular units (FUE) as a clinical research project. And he eventually realized that the various mechanical systems he had developed did not produce the continuous extraction of fine follicular units per patient. Because he used suction remove the graft.

The variable results were not related to any of the specific mechanical systems developed by the authors. The author completed a number of patients with FUE in the late 1990s. In 2002, William Rasman and partners reported that there were histological differences between follicular units that you could easily remove them or not.

Surgeons have identified a variety of histological elements that appear to be difficult to “remove follicular units”; including a thick skin sheath and less elastin in the skin sheath, possibly related to unit nucleation resistance.

they labeled patients who had difficulty nucleating with a negative FOX (FOX-) label; and labeled those with easy nucleation with a positive FOX (FOX +) label; and there were many patients who were clearly not FOX + and not FOX – they were just something in between. Surgeons suggested that each patient could have a set of experimental transplants to determine how easy the transplants were.

If there was a complete design for a mechanical system, it would have to extract a fixed quality link independent of the patient’s FOX status. This challenge quickly became apparent. In 2003, Rasman and Pack designed and patented a robotic system for locating and automatically extracting a follicular unit. This invention was patented in 2006; and we can say that it might be more suitable for FOX- patients. This design is included in today’s Artas robotic system.

The history of hair transplant in Iran

As you know hair transplant is a very popular surgery in Iran; and after rhinoplasty it sits in the second place of popularity. The high amount of requests for this surgery has led to low percentage of mistake in surgery, skillful surgeons, satisfying results and high quality services.
