The doctors of Gangaram’s Department of Plastic and Cosmetic Surgery have not only reattached three fingers cut from the root of that person’s hand, but have also amputated a toe and attached it in place of the thumb. Interestingly, this reattached thumb is also fully functional.

Recently, a 44-year-old man from Uttarakhand was brought to the emergency at Gangaram Hospital for treatment. Three fingers and a thumb of his left hand were severed from the root. This accident happened while working in a factory in Uttarakhand. It took eight hours for the Patient from Uttarakhand to reach the hospital and he was bleeding profusely. The patient brought with him three cut and crushed fingers in a polythene bag but the thumb was missing.

Dr. Mahesh Mangal, Chairman, Department of Plastic and Cosmetic Surgery, Gangaram, told that the challenge for us was not only to join the three crushed fingers back to the hand, but the biggest challenge was to reconstruct the thumb that was not amputated. For this we decided to convert the second toe of the patient’s right foot into a thumb so that his hand could be fully functional. This work was very difficult and challenging.

For this, a team was formed under the leadership of Dr. Mahesh Mangal, in which Dr. SS Gambhir from the Department of Plastic and Reconstructive Surgery, Dr. Nikhil Jhunjhunwala, Dr. Pooja Gupta and Dr. Manish Dhawan from the Department of Orthopedics were included. . After that the patient was immediately taken to the operation theater and a 10-hour long microsurgery was performed. All the three fingers were reattached by microscopic surgery to remove nerves, arteries and crushed bones and tendons. Since the thumb was not brought by the patient, we decided to cut off the second toe of the patient’s right foot and attach it to the thumb of his left hand by another operation. This operation was very complicated and long, it took about eight hours.

500 organs have been transplanted in the hospital
Dr. Mangal said that microsurgery was started in Gangaram Hospital in 1981 in the Department of Plastic Surgery. Since then the department has become a center of excellence in replanting body parts amputated due to industrial, agricultural, domestic, road traffic accidents etc. So far 500 re-transplantations have been done of various amputated body parts like fingers, toes, penis, skull, ears, upper limbs, etc.

He told that not only this patient, but two other similar patients also came to the hospital in the same week. One of the patients was a 10-year-old boy who had come from a village in Rajasthan. He had a cut finger. He got hurt while playing with the woodcutter. Since the level of dissection was too far apart and the patient was a child, there was a need to connect blood vessels. Due to young age, these nerves were very fine. In such a situation, super microsurgery was done to reconnect his finger. This operation was successfully done in seven hours.

On the other hand, the second patient was a 20-year-old male from Noida, whose right finger was amputated. He was injured at home due to accidental fall of the wash basin while cleaning and was brought to the emergency in the middle of the night. After 6 hours of microsurgery, his severed finger could be attached back.
