Touchmark Home Health is part of Touchmark at Meadow Lake Village’s leading full-service retirement community. Our goal is to enrich the lives of residents and off-campus clientele by providing them with exceptional service in the comfort of their own homes. We are currently seeking the right person to join our team as a Home Health Physical Therapist.

Wage starting at $49 per hour

As a Home Health Physical Therapist (PT) you will enrich the lives of our clients in comfort of their own home in the Treasure Valley.

As a Home Health Physical Therapist You Will

• Review physician referrals and create a personalized physical therapy treatment plan that sets realistic and achievable goals for clients in a home setting;
• Record and document patient condition, education, and physical progress;
• Work with interdisciplinary team to develop plan of care;
• Create and monitor PT treatment plans for physical therapy assistants;
• Work independently in client homes using your good judgement and critical thinking skills to best serve the patients;
• Have transportation to client homes throughout the Treasure Valley area.
  
  

To Successfully Serve Our Patients You Will Have

• Current Physical Therapist (PT) License in good standing in Idaho
• Current CPR & First Aide certifications
• Transportation to client home, current drivers license, vehicle insurance, and complete and follow Touchmark Driving Policy
