About the role

This position is within the dynamic, multidisciplinary Community Rehabilitation Team and involves the provision of clinical case management services to complex clients including those with an acquired brain injury and complex neurological conditions.

Key Responsibilities

• Comprehensive assessments and plans
• Assisting clients to develop and met their rehabilitation goals
• Coordination of services and referrals for the complex client
