To help fix an issue Triple Pack owners experienced after downloading update 1.08 yesterday, Naughty Dog released Uncharted 4 update 1.09 today, which is 70MB in size.

Naughty Dog’s Drew Thaler said:

Uncharted 4 1.09 is out, and fixes the Triple Pack. If you were waiting for those awards to pop go check now!

In the feedback thread about Triple Pack extras being locked, Naughty Dog said:

Please patch to 1.09. Your triple pack should then unlock. We are aware users in Europe are continuing to experience this issue and are working with PSN to resolve it.

Thaler added, “Sounds like there are still Triple Pack problems in Europe — I am having *words* with *people*, we’ll get it fixed for you.”

Since all multiplayer content in Uncharted 4 can be earned through gameplay, Naughty Dog removed the Triple Pack from the PlayStation Store in Europe yesterday and replaced it with the Explorer’s Pack. In North America, the Triple Pack is still available.
