A 48-year-old woman who has been receiving treatment for obesity is having a follow-up visit 9 months after starting subcutaneous semaglutide 2.4 mg weekly in addition to lifestyle modification. Her body mass index (BMI) at the start of the therapy was 34 kg/m2, she had mild hypertension treated with hydrochlorothiazide, and you counseled her about the cardiometabolic benefits of losing 10% or more of her weight at that time. Since then, she has introduced recommended changes to her diet, slightly increased physical activity, tolerated semaglutide well, and lost 11% of her weight; her current BMI is 30 kg/m2. Her blood pressure has been normal in the last 3 months without hydrochlorothiazide. She is happy with her progress and wishes to stop semaglutide 2.4 mg weekly now. What would be the best approach to therapy and counseling at this time?
