Research indicates that ADHD symptoms can persist into adulthood, significantly impacting relationships, careers, and even the personal safety of patients suffering from the disorder.
As the condition is commonly misunderstood, many individuals who have it do not receive appropriate treatment, and as a result, may never fully develop their potential. Part of the problem is the difficulty in diagnosing, particularly in adults.
This questionnaire is intended to stimulate dialogue between the doctor and the patient and to confirm the presence of disorder symptoms. The questionnaire’s content assists in uncovering symptoms, impairments, and history that could lead to the correct diagnosis. It does not replace an evaluation with a specialist doctor.
This questionnaire is merely a starting point for identifying some possible primary symptoms of ADHD.
The correct and accurate diagnosis of ADHD can only be made through an extensive anamnesis (interview) with a specialized medical professional (psychiatrist, neurologist, neuropediatrician).
Many of the symptoms listed below may be associated with other comorbidities related to ADHD and other clinical and psychological conditions.
Always remember that any diagnosis can only be provided by a medical professional.
