Global Delay Development (GDD) is an abnormality of motor function, mental function and postural tone that is acquired at an early age, even before birth. This abnormality in the motor system is the result of brain lesions that are non progressive. The motor system of the body provides the ability to move and control movements. The abnormality in these systems means that it affects the Childs ability to perform activities of daily activities and affects their ability to move and walk.

Dr Nandini Gokulchandran, Deputy Director and Head Medical Services, NeuroGen Brain and Spine Institute said “Signs and symptoms of Global developmental delay usually show in the first year of life. The predominant symptoms and signs of GDD are related to motor difficulties, which are the consequence of the brain damage. The extension and severity of the brain lesion is the leading factor in the magnitude of the motor deficit. Many of the symptoms observed in these children are related to the primary problem that is impaired motor functions. Some of the symptoms a child may display would include developmental motor delay, gait disorders, poor fine and gross motor coordination, swallowing disorders, or speech delay are all the result of the basic motor disorder. GDD affects approximately one to three out of every thousand children born. However, it is much higher in infants born with very low weight and in premature infants. Most of the causes of GDD do not have specific, curative treatments.”

Dr Nandini Gokulchandran further said “However, children with GDD present many medical problems that can be treated or prevented. The initial stage of treatment involves an interdisciplinary team, consisting of a pediatrician, preferable one with experience in neurodevelopment disorders, a neurologist (or other neurological practitioner), a mental health practitioner, an orthopedic surgeon, a physical therapist, a speech therapist, and an occupational therapist. Each member of the team has important, independent contributions to make in the care of the affected child.”

Dr Nandini Gokulchandran said “Although, these treatment option help improve the quality of life for these patients, they do not solve the problem to its core. Modern medicine and research have together thrown light on the potential use of stem cells for curbing the disastrous effects seen in GDD.”

Today we are presenting a case report of Hiba Khan, who is a 5 years old female and a known case of Global Developmental Disorder with Microcephaly, with no family history of it or other psychological disorders.

Hiba was born a full-term delivery by cesarean, due to mucus congestion in lungs. She did not cry immediately after birth. All her motor milestones were delayed. She was taken to Children’s specialist at around 1 year of age and due to frequent illness where she was recommended to undergo a MRI. This suggested some deformity in brain leading to developmental delay. Hiba’s parents consulted two to three doctors but no one suggested any treatment for the same. At the age of 1.5 years, Hiba started suffering a series of seizures. The first one lasting for about 5 mins was a silent seizure during which she was fisting with froth coming out of her mouth followed by bluish coloration of lips. She was unconscious for 8 hrs post this episode. Physiotherapy was started along with some anti-epileptic drugs. Such occurrences kept on happening with a total of 4 more episodes with almost the same outcome and condition.

When Hiba’s parents learnt about that their child can be treated through stem cell therapy they immediately consulted the doctors from NeuroGen, sent reports to the doctors through telephone and e mail, and when they received a positive response from NeuroGen experts that Stem Cell Therapy followed by Neuro Rehabilitation can help their child, Hiba visited NeuroGen in November 2017 for treatment.

No awareness. Could not imitate actions and gestures. Could not receive and expressed emotions appropriately. Poor in social interaction, could not pursue basic level relationship.Could not speak.Poor communicative skills.Could not understand Verbal & Nonverbal cues.Could not convey her needs.She was dependent for All Day Living Activities such as eating, dressing, etc. She was not toilet trained.

At NeuroGen Brain and Spine Institute, Hiba underwent Stem Cell Therapy along with a customized rehabilitation program. The aim of the rehabilitation program was to develop cognition and understanding, therapies to reduce the hyperactivity and to increase the concentration, command following and to increase overall intellectual ability of the patient. She was given exercises that would help her improve understanding, address her behavior, sensory and motor issues. She was given a combination program with occupational therapy, physiotherapy, speech therapy and psychological counseling under the guidance of the most experienced professionals in the field. This helped her to help develop her overall skills, abilities and overall development. These exercises carried out in a systematic pattern with sufficient rest intervals to the patient. Together, the aim of the rehabilitation program was to improve her overall quality of life. Occupational therapy and rehabilitation taught at NeuroGen were continued after going home.
