Q&A : Stokes-Adam syndrom and its arrhythmical mechanisms

Q : Doctor ,  In stokes-Adams Syndrome the ventricles fail to pump blood because the sudden blockage of a-v bundle, and the Purkinje fibers had been over driven by the rapid sinus impulse which leads to suppressed state and person faints after 4-5 seconds.....why the ventricle fail to pump blood if it has over driven rhythmical impulse from the rapid sinus impulse??!

A:  Stokes - Adam syndrome is a Disease that causes syncope and loss of consciousness without warning . Many conditions are involved in the disease . Some of them :
1. Congenital structural lesions in the heart.
2. Myocarditis 
3. connective tissue diseases 
4. Diphteria toins
5. Myocardial infarction 
6. Fibrosis of the excitatory conductive system .
7. Paget`s disease , amyloidosis , Reiter`s syndrom .
8. hyperkalemia
9.acidosis 
10. Drugs , like procaineamide and quinidine .
Whatever the cause , In Stokes- Adams syndrome , cardiac arrhythmia will develop.  
There are many types of arrhythmia that may be underlying the syncope of  Stokes-Adam syndrome :
1- Atrioventricular block : Please pay attention : When the cardiac rhythm is changed from sinus rhythm into complete AV block , A period from 10-90 seconds of asystole ( loss of systole ) develops , then an ectopic junctional or ventricular assume the rhythm . 
2- Bradycardia (slow heart rhythm ) : Failure of the SA node to generate a rhythm  , and then the AV node will not transmit the rhythm to the ventricle , and thus a period of asystole will develop too.
3- Paroxysmal tachycardia or fibrilation : This tacharrhytmia originates in ectopic foci , and would not produce adequate contraction of the ventricular muscle , so the syncope will develop .
The Mechanism you have mentioned in your question is similar to the third mechanism . But remember: This is not the only arrhythmical mechanism in Stokes-Adam syndrome .
