This is classic example of hump or bump removal in Rhinoplasty surgery. This young lady from San Diego decided to undergo cosmetic nose reduction surgery to reduce the dorsal hump or bump.

This happens to be one of the most common types of nose jobs that Dr. Hilinski performs. Reshaping of this type of nose can be done with either an open or a Closed Rhinoplasty Approach. The former is used more frequently by Dr. Hilinski because he prefers to see the actual anatomy before and after removal of the nasal bump.

Here is the actual intraoperative photo of this patient once the hump was exposed during rhinoplasty surgery by Dr. Hilinski. And the actual hump is shown as it was exposed by lifting up the skin over framework of the nose. In this particular rhinoplasty patient, the hump was a combination of Cartilage and bone. Although this type of bridge reduction could certainly be done without exposing the cartilage and bone (using a closed rhinoplasty approach), Dr. Hilinski ended up reinforcing the leftover cartilage. This specific maneuver necessitated the open approach as is seen here.

Once the cartilage and bone were exposed fully, removal of the bump was started by cutting out the excess cartilage. That specific maneuver is shown here where Dr. Hilinski began using a scalpel to cut out the cartilage. Once the cartilage was cut out and a good portion of the hump was removed, the bony segment was then sanded down using a surgical rasp. This is what many patients are familiar with already where the rhinoplasty surgeon’s hand is moved back and forth while holding the rasp. This essentially allows the surgeon to sand down the bone to achieve the desired bridge height. After this is done, the bones are then broken and narrowed so that the bridge is not left excessively wide.

And here are the actual results of this rhinoplasty patient. As you can see, her bump has been removed very nicely to give her a straighter profile. In addition, her nasal tip was refined a bit to give her a better looking frontal view. Yet – overall – she still looks like herself – the goal of nearly all rhinoplasty surgeries.
