The Cumulative Injury Cycle

When it comes to injuries, they can generally be broken down into two categories: acute and chronic. Acute injuries are those that are severe with a sudden onset. These injuries can include trauma injuries. Chronic Injuries occur over the course of time and are prolonged. In the field of physical therapy and corrective exercise, these are considered overuse injuries. Acute injuries can become chronic, and it is debatable as to whether all chronic injuries began as acute. The semantics of this debate will not be discussed here. What will be discussed is the model for the cumulative injury cycle. The cumulative injury cycle can be used to explain how acute injuries become chronic, as well as how overuse injuries occur. It is recommended that prior to reading this article one has a basic understanding of the healing response

Stages of The Cumulative Injury Cycle

Inflammation stage

In most cases the cumulative injury cycle begins with an acute injury. The sprain of an ankle, muscle tear, etc. The healing process begins with inflammation, then forms the building block for tissue and tears and microtears begin to close in the proliferation phase. Then the tissue grows, becomes flexible and matures in the remodeling phase.

The initial injury and inflammation cause muscle spasm. This causes the myofascial tissue of the muscle to sometimes stick together. The myofascial tissue may also become attached to the healing site of the injury. As the injury bed is becoming remodeled and the tears and microtears begin to close due to the filling of epithelial tissue; the sticky myofascia can sometimes get caught in the matrix. The result of this is the build up of a knot of tissue.
