Combination Therapy: Help When Viagra, Alprostadil Alone Don’t Work

Most men suffering from erectile dysfunction respond to treatment with an oral ED drug such as Viagra or Cialis. Those who don’t can usually find relief through penile injection therapy. However, when monotherapy fails, combination therapy may be the answer.

The introduction of Viagra in the late 1990s heralded a new era in the treatment of erectile dysfunction. Pfizer’s little blue pill and the similar medications that have followed it to market represent the treatment of choice for millions of men with ED.

As welcome a development as this has been, it has not brought relief to all men suffering from symptoms of impotence. For some of those who haven’t responded to treatment with Viagra or other oral ED drug, another option has been Penile Injection Therapy.

Alprostadil Offers Another Option

Older than the oral ED medications, this form of treatment involves injection of a vasodilating drug — usually Alprostadil — directly into the shaft of the penis. This therapy has an even higher success rate than the oral medications but is avoided by those reluctant to stick a needle into their penises.

It should be noted that alprostadil can also be introduced into the penis in the form of tiny drug-containing pellets inserted into the urethral opening at the very head of the penis or as a topical application.

Impotent men who fail to respond to either of these forms of treatment often resign themselves to a life without sex — or at least conventional sex. Forunately, recent research indicates that there is hope for such men in the form of Combination Therapy.

Combination Therapy Evaluated

A team of Spanish researchers looked into viable alternatives for ED sufferers who had failed to respond to monotherapy — the use of a single drug to treat a disease or chronic disorder. Their findings were published in a 2018 issue of the International Journal of Impotence Research.

Their review of relevant scientific literature found that combination therapies involving the use of an oral PDE5 inhibitor such as Viagra together with alprostadil produced significantly better results than the use of monotherapy. Their findings held true whether the alprostadil was injected directly into the penis or as a topical or intraurethral application.

This is a significant development for men who have failed to respond to monotherapy with a PDE5 inhibitor or alprostadil alone. Using these vasodilating drugs in combination seems to provide the extra pharmacologic boost men need to temporarily regain near-normal erectile function.
