How to fill Form 60 step by step of Bank of Baroda

Bank Account Holder have to produce PAN Card if they want to deposit a minimum sum of Rs 5000 into their Account. In case if bank account holder doesn’t have PAN Card or Haven’t Received yet then in that case, the holder has to fill form 60 and submit in replace of PAN Card for making transaction worth 50000. Form 60 comprises of basic details about Account holder and also about the type of transaction that Account holder is making. Account holder also needs to submit one Identity and address proof document, whose details have been mentioned in the form

How to fill Form 60 of Bank of Baroda

If you are Bank of Baroda, and making deposit transaction of Rs 50,000 then here the guide on filling form 60 in case of absence of PAN Card.

Column (1-11) Account holder needs to furnish basic details like his Full Name, Date of Birth, Father’s Name, Residential Address.

Column (12-15) Account holder needs to mention his State, PIN Code, Telephone number ( if any) and the most important Registered Mobile number.

Column 16 ( Amount of transaction) Write the value of transaction you are making for example, if you are depositing Rs 50000 then mention here Rs 50000

Column 17 (Date of transaction) Write the date of transaction when you are making this transaction

Column 18 In case you are making this transaction in joint name then write all the person involved in the transaction.

Column 19 (Mode of Transaction): Choose the appropriate mode by how are depositing the amount of transaction. For example, if you are depositing Rs 50000 by cash then tick on cash, if depositing by cheque then tick cheque.

Column 20 (Aadhaar Number) Enter your 12 Digit Aadhar number issued by UIDAI

Column 21: If you have applied for PAN but still haven’t received yet then it that case enter the acknowledgement number mentioned in PAN Card receipt.

Column 22: In case if still not applied for PAN Card then the account holder needs to furnish information about his total income for the financial year he is making the transaction.

a. Agriculture Income: In case of income attained from agriculture, total Yearly income earned must be entered here.

b. Other than Agriculture Income: If your income earned is from other sectors rather agriculture sector then mention the total income earned in current financial year.

Column 23: Account holder needs to provide details of document he will submitting as Identify proof in support of Form 60. Account holder can submit any Document whose list is given at bottom of form as proof of identity as accepted by Bank.

Column 24: Account holder needs to provide details of document he will submitting as Address proof in support of Form 60. Account holder can submit any Document whose list is given at bottom of form as proof of address as accepted by Bank.

VERIFICATION 

I, ____Write the Account Name____ do hereby declare that what is stated obove is true to the best of my knowledge and belief. I further do not have a Permanent Account Number annd my/our estimated total income (including income of spouse, minor child etc. as per section 64 of Income-Tax, 1961) computed in accordance with the provision of income -Tax Act, 1961 for the financial year in which the obove is held wil be less than maxilmum amount not chargeable to tax.

At last, attach one zerox copies of the Identity and Address Proof documents as mentioned in the form.
