1. O’level result certificate and photocopy
2. Evidence of Jamb Result or Jamb Regularization
3. Birth Certificate/Declaration of Age
4. Local Government of Origin
5. Evidence of Change of name (if any)
6. Evidence of Industrial Training (HND Only)
7. National Diploma (ND) Notification of result and photocopy (HND Only)
7. Any other relevant document.
8. Pay Acceptance Fee
