What makes a good test plan – Part 1

We’ve been having a discussion recently within the business about what makes a good test plan.  We’ve written many and most have varied quite a bit.  The content should be directly related to the circumstances and to the expected or desired outcome.  But what should make up the core document?  Is it a living Document and how should we be tracking that the most up to date documents is the one being shared or read by the close and wider audience?
