Description

The Monitoring Specialist works cooperatively with Supervisors in the Department of Personal Supports and Opportunities to conduct monitoring activities in host home settings. Monitoring activities include face-to-face monitoring and review of documentation. The Monitoring Specialist supports host home providers in resolution of problems found while monitoring, including providing applicable training as the situation may dictate. The caseload will include individuals supported in host homes.

JOB FUNCTIONS:

1. PROGRAMATIC. Promotes and fosters a supportive and person-centered atmosphere for individuals residing in host homes and family care giver settings. Supports providers and staff to ensure that individuals have opportunities to exercise choice and pursue personal goals. Ensures that individuals are involved in all aspects of the daily routine in their homes. Assures that individuals receive all services and supports identified in their Service Plans and required by regulation including implementation of Individual Service and Support Plans, Behavior Support Plans, nursing care plans/protocol, therapy programs/protocols and other identified services and supports. Participates in the development and implementation of new program initiatives and attends team meetings. Trains host home providers on how to document, navigate our database, and on any necessary documents required for the individual.

2. HOME INSPECTIONS/ENVIRONMENTAL HEALTH AND SAFETY. Ensures that HUD inspections are completed for each prospective host home or Family Caregiver setting. Follow-up to assure that any deficiencies noted in HUD inspection are corrected prior to initiation of contract. Ensures that the home meets the needs of the individual, including but not limited to safety and accessibility. Ensures that needed training, retraining, and support is provided to Host Home Providers.

3. RESIDENTIAL BACK-UP. In conjunction with the Host Home Coordinator, assures that host home providers comply with requirements related to use of residential back-up services including assuring that host home providers:

• Use qualified back-up providers.

• Provide notice of use of back-up providers.

• Adhere to requirements regarding payment of back-up providers.

4. HOST HOME MONITORING. Conducts host home monitoring visits in accordance with established schedule and as assigned. All scheduled visits to be scheduled a month in advance and reminder call a week prior to visit. Completes record review prior to monitoring visit and documents. While onsite, monitors all areas noted on checklist and documents the results. Documents any additional observations/concerns. Will complete unannounced visits throughout the year. Provides support to host home provider to resolve any noted problems. Provides completed monitoring forms to Designated Host Home Coordinator. Reports serious problems immediately, including any imminent risk to the health and safety of the individual.

5. ASSESSMENT: In collaboration with other team members and the individual, completes the annual assessment processes. Identifies the individual's unique strengths, support needs, goals and desires. Presents pertinent information at the annual Service Plan (SP). Develops and/or updates residential IPOP, assuring that all information is current.

6. REPORTS/THERAP MONITORING: In collaboration with other team members, completes weekly and monthly reports set forth by Residential Director. Reports and monitoring are documented and turned the first of every month.

7. EMAR UPDATING & MONITORING: Monitors individuals EMAR's weekly for prompt and correct documentation. Follows up with host home providers and team when medication is missed or refused. Also, coordinates with the individual's host home providers, their medical professionals, and Strive's nursing department to update their EMAR.

8. APPOINTMENTS: Sometimes assists the host home provider and individual with making appointments with medical professionals and attends when needed. Monitor keeps track of annual appointments and ensures proper documentation is provided.

9. ON-CALL: Monitor is be put into a rotation to take on-call for a week each month. Monitor will be responsible for holding on-call phone to answer questions from staff or providers. You may be asked to cover a group home shift if staff are not available as well as respond to emergencies.

10. SCHEDULING: Monitor may be responsible for scheduling and getting shifts covered by support staff for host home providers who receive extra support through our Host Home Plus program.

JOB REQUIREMENTS/SPECIFICATIONS:

Education/Experience/Skills/Knowledge/Licenses:

• 21 Years of Age.

• H.S. Diploma or GED.

• Able to effectively use a computer, familiarity with Microsoft Word.

• Valid driver's license and driving record that meets STRIVE standards

• Personal vehicle for work-related travel.

• Excellent problem-solving skills.

• Effective time management skills.

• Effective Communication

• Writing Skills

$17.85 Per Hour
