In the past, I have used FormSpring for multiple sites and organizations; utilizing both their free and paid accounts.  Among these applications have been contact forms to reduce spam, mailing lists, event registration, volunteer registration, secure donation collection and legislative advocacy campaigns.  In three years of working with FormSpring we have not had one complaint.  Moreover, FormSpring offers an affiliate program than can help offset the costs of their service or provide additional revenue to online marketers.

Formspring's affiliate program offers you the opportunity to earn a 25% commission off of any customer you refer, during the first year of their account.  Moreover, referrals do not even have to immediately sign up for a paid account to earn a referral fee.  If a customer signs up for a free account and then converts to a paid account 4 months later, you earn your referral fee for the remainder of the first year.  

The referral link utilizes a 30 day cookie and payments are sent within five days of the first calendar day of the month.
