SLAP LESION

Injuries to the glenoid labrum which is the fibrocartilagenous rim that is attached around the scapular glenoid cavity are known as SLAP tear or SLAP lesion. This is known as superior labrum anterior to posterior tear as a superior portion of the labrum tends to get torn in this lesion. This lesion involves the injury to the biceps tendon as it takes origin from the superior labrum.

There are four types of SLAP tears:

Type 1: The bicep tendon only gets frayed degenerative in this and the attachment of the tendon is intact.

Type 2: This is more common. In this, the biceps tendon gets detached along with the superior labrum from the glenoid rim

Type 3: in this the biceps tendon attachment is intact but a buckle handle type of tear of the labrum is evident.

Type 4: This is uncommon, which involves both superior labral buckle handle tear and the bicep tendon detachment from the glenoid rim

RISK FACTORS

• Age above 35 years
• repetitive throwing,
• hyperextension,
• a fall on an outstretched arm,
• heavy lifting,
• Direct trauma.

CLINICAL PRESENTATION

• Painful clicking and/or popping during shoulder movement
• Glenohumeral internal rotation range of motion loss
• Painful overhead motion
• Rotator cuff muscular strength and endurance loss
• Scapular stabilizer muscle strength and endurance loss
• Inability to lie on the affected shoulder

Athletes who are involved in overhead sports like badminton, tennis, volleyball, pitchers develop ‘dead arm’ syndrome in this they have pain during throwing activities and they’re no longer able to throw at their pre-injury velocity, accuracy, and level.

MANAGEMENT

Medical management

Type 1: Debridement.

Type II: Arthroscopic fixation of the superior labrum

Type III: Aebridement by an arthroscopic shaver

Type IV: Repaired with multiple sutures

Physiotherapy management:

• Regaining GIRD
• Posterior capsule stretching exercises
• Exercises for scapula stabilisation
• Strengthening exercises: forward flexion in a side-lying position, prone extension, seated rowing, serratus punch (protraction with the elbow extended), forward flexion in external rotation and forearm supination, full can (elevation in the scapular plane in external rotation.

If conservative treatment is failed, surgical treatment is recommended and physiotherapy should be initiated as early as possible postoperatively.
