Preface

Nothing stays the same. Politics, art, science, technology, the environment—if there is one constant in life, it is the inevitability of change.
In the last half century, the speed of that change has increased at a pace unique in human history. We live in an era that has been called “the age of acceleration,” in a rapidly evolving world that increasingly delights in pulling the rug out from under us. Just when one starts to feel comfortable with a predictable routine, life imposes a new reality. Change is as inescapable as it is impenitent.
Without a doubt, this shift in momentum has affected the field of medicine. A look at the table of contents from the last edition of Abrams’ Angiography, released in 2006, reveals that interventional radiology has kept pace. The hot topics and procedures at the time of publication were on the leading edge of medical innovation. In less than a decade, these advances would be eclipsed by new interventions of startling impact and clinical benefit. The field changed so quickly that many of these imageguided less invasive procedures were completely unanticipated when the last volume of Abrams’ was released.
This rapid evolution makes sense. Interventional radiology has always been about what’s next, with deep roots in the future. Today, just as when the 1st edition of Abrams’ Angiography was introduced in 1961, innovation and interventional radiology remain synonymous. Change through innovation is where the early interventional pioneers planted their flag and that is where the field continues to grow and prosper.
As editors of this new edition of Abrams’, we felt compelled to honor this legacy of innovation. Thus, the focus and content of the book have been completely revamped to reflect the fast-moving landscape of the practice of interventional radiology. The organizational format of this volume, including chapter titles and authors, was created from scratch with the goal of providing a fresh take on leading edge subjects by the most knowledgeable current authorities.
Traditionally, one of the goals of a standard reference text is to be a comprehensive compendium of a given field. We took on that challenge in this edition of Abrams’, but we also intentionally pruned its scope to 100 chapters—what we consider the most salient top 100 topics that are key to mastering contemporary interventional radiology. Obviously, this means some aspects of the field will receive less emphasis. We acknowledge the arbitrary nature of our chapter selections, but in the end we believe it is important to limit the project to one relevant volume.
One obvious change over the last decade has been a crescendo of interest in the area of interventional oncology. This important aspect of contemporary interventional radiology is a natural extension of the percutaneous and endovascular procedures that were initially applied to hepatic malignancies, but were only briefly described in the last edition.
In this latest edition, we devote about one-third of the content to topics directly associated with interventional oncology. Two-thirds of the text addresses the management of arterial and venous disease. Many of the themes considered in the vascular sections are rooted in the original stock of the initial Abrams’ volume. All of the new chapters, however, focus on the current understanding and management of vascular disease.
In regard to authors, we have been blessed with the wisdom and talent of the best and brightest experts in interventional radiology. We take this opportunity to express our deepest appreciation to these friends and colleagues. Their contributions captured the groundbreaking achievements and vibrating excitement of interventional radiology today. It has been an honor and privilege to work with them on this text.
