In short, when you’re low on testosterone, your penile nitric oxide synthase will be low. Then your corpus cavernosum won’t become filled with blood. Then you won’t have an erection at all. Or your erection would be too weak to sustain penetrative sex.

In addition, testosterone is also important for your erectile function because the hormone helps regulate the neural mechanisms involved in penile erection. The nerves located in your genital area are dependent on testosterone when they process signals for sexual stimulation and arousal.

In animal studies, it has also been shown that testosterone plays an important role in maintaining the structure and organization of the cells in your corpus cavernosum. When the structure or organization of your corpus cavernosum cells are altered, it can result in an impaired erectile function.
