How to correct mistakes in Pan card form by filling Pan card correction form

Hello in this article we will Learn how to correct you mistakes in your Pan card by filled pan card correction form. This method is offline where you need to fill up a form pan card correction form and then you need to submit this form with correct valid documents to the nearest Pan card office with a fees of Rs. 120. Note**: You need to fill this pan card form in English BLOCK letters only and you have to use Black ball point pen to fill this form.

Full Description of each coloumn of Pan Card form:

First of all, you need two same recent size passport photograph. At the top extreme left hand cornor of the pan card form in the Box you need to paste one of your photo and you also need to sign across the photo in such a way that half of your signature is in your photo and half outside your photo. Now at top extreme right hand cornor of the pan card form in the Box you need to paste your second passport size photo and here you need to do your signature in running hand( just write your full name) just below the Box where you have pasted your photo. Make sure the photograph you are using should not be the scanned photograph or your application will be rejected. (See Obove screenshot)

Permanent Account Number ( PAN )

Just write your Existing 10 alpha-numeric PAN Card Number here in this coloumn. PAN Card number can’t be wrong

1 Full Name (Full expanded name to be mentioned as appearing in proof of identity/address documents: initials are not permitted)

Please select title: Just select the prefix or title that you use before your name such as Shri, Smt, Kumari, etc.

 Last Name / Surname: Just write your surname that is the name, you use after your first name.

 First Name: Write your first name, For eg, Ravi

 Middle Name: Write your Middle name,if any, for example, Ravi KUMAR Singh, here KUMAR is middle name.

Name you would like it printed on the PAN card

Just write the exact name in English capital letters which you like to be printed in the pan card as your name.

2 Details of Parents (applicable only for Individual applicants)

Father’s Name (Mandatory. Even married women should fill in father’s name only)

Last Name / Surname: Just write your father Last name or surname.

First Name: Just write your father first name. For example, Ravi Kumar Singh, here Ravi is first name.

Middle Name: Just write the middle name( if any). For example, Ravi Kumar Singh, here, Kumar is middle name.

Mother’s Name (optional):

Last Name / Surname: Just write your mother last name or surname.

 First Name: Just write your mother first name. For example, Sarla Devi Singh, here Sarla is first name.

 Middle Name: Just write the middle name( if any). For example, Sarla Devi Singh, here, Devi is middle name.

Select the name of either father or mother which you may like to be printed on PAN card (Select one only)(In case no option is provided then PAN card will be issued with father’s name)

In this coloumn you need to tick in the box of either Father’s Name or Mother’s name so that apart from your name your father or mother name is also printed. It has now been made mandatory that you have to choose either of the two option. In case if you do not tick on any option then automatically your father name will be printed in the pan card apart from your name.

Date of Birth/Incorporation/Agreement/Partnership or Trust Deed/ Formation of Body of individuals or Association of Persons

Just write your date of birth in the format of DD-MM-YYYY in the boxes in case of individual category but in case of partnership or Agreement then you need to mention the date when you signed the contract or the date when partnership deed was signed and formed.

Gender (for Individual applicants only) Male Female (please tick as applicable)

Just choose your  gender Either Male or Female

Photo Mismatch: Tick on this coloumn, if your photo mismatches in the pan card with the one who submitted in the form at the time of applying

Signature mismatch: Tick on this coloumn, if your signature mismatches in the pan card with the one who submitted in the form at the time of applying

7 Address for Communication: Just tick the address either office or residence on which you would like to receive you pan card delivery.

Office Address: If you are employed then mention the address of your office.

Flat / Room / Door / Block No: Just write your current address. For example, 54/K, Vishal Nagar, Here,54/K is your house number.

 Name of Premises / Building / Village: Just write the name of the Building if it has name in case of complex or Flat name.

 Road / Street / Lane/Post Office: Just write the name of the area or street where your residence is established or nearest Post office near your house as your landmark.

 Area / Locality / Taluka/ Sub- Division: Just write the name of the colony or area where your residence is established.

 Town / City / District: Just write the name of the city where your live

State / Union Territory: Just write the name of your State where you live
 Pincode / Zip code: Just write your area PIN Code. For example, 226012
 Country Name: Just write here INDIA.

8 If you desire to update your other address also, give required details In additional sheet.

If you want to update your address also then you need to provide your new address supporting a valid document in seprate form.

9 Telephone Number & Email ID details

Country code Area/STD Code Telephone / Mobile number/Telephone Number: Just write your mobile number.
& Email ID details: Just write your email ID (not mandatory).

10 AADHAAR number (if allotted): Just write your 12 digit Aadhar Card number

Name as per AADHAAR letter/card:
First Blocks: Write your First name as per your aadhar card
Second Blocks: Write your secondname as per your aadhar card
Third Blocks:
Write your third name as per your aadhar card

11 Mention other Permanent Account Numbers (PANs) inadvertently allotted to you

If by mistakenly you have been allotted more than one PAN Card number then you can just surrender the additional PAN Card number by writing in this coloumns.

PAN 1: Surrender PAN Card number ( Additional)

PAN 2: Surrender PAN Card number ( Additional)

PAN 3: Surrender PAN Card number ( Additional)

PAN 4: Surrender PAN Card number ( Additional)

12 Verification

I/We……Just write your name in case of individual category and write the name of company on case of business.……..,the applicant, in the capacity of………write self or your name in case of individual category and in case of Business write as partner or in case of Head of family write as Karta or write Representative assesse when the form is been filled by another person apart from the details whose mentioned in the form………do hereby declare that what is stated above is true to the best of my/our information and belief.

I/We have enclosed (write the number of documents you are submitting as a proof of correct detail in your Pan card) in support of proposed changes / corrections.

Place: Write your city name where you recide
Date: Just write the date in the format of DDMMYYYY when you are filling this form.

Signature / Left Thumb Impression of Applicant (inside the box): Just do your signature or thumb impression in this place. Do your signature in running hand like writing your full name rather than doing like signature given to bank
