The Occupational Safety and Health Administration (OSHA) is investigating two separate explosions that occurred at Archer Daniels Midland (ADM) corn processing facilities in Illinois and Iowa on January 5.

One firefighter was killed and another was seriously injured in the explosion and resulting fire at the company’s Plant in Clinton, Iowa. The firefighters arrived at the scene after ADM employees found smoldering material in a silo storage bin at the facility.

OSHA has six months to determine whether health or safety hazards were partially responsible for the incident. The agency will issue citations if any violations are identified.

Hours before the Clinton incident an explosion occurred at the ADM complex in Decatur, Illinois. No injuries were reported. It came just two months after another explosion and fire in the grain elevator used at the company’s corn and soybean plants.

ADM has been under investigation in the past for various workplace accidents. In May 2018, a Clinton plant employee was seriously injured when a boom truck came into contact with a high-voltage power line. OSHA classified the incident as a serious violation and issued a $12,471 penalty.
